Located in Palm Coast, FL, our dedicated team of dentists near you sets aside 45 minutes to an hour to get acquainted with you and provide support on your first appointment. We kindly request that you provide a 24-hour notice in the event you are unable to keep your appointment. 

What to Bring With You

  • A list of all prescribed and over-the-counter medications you are currently taking.
  • Any dental insurance or discount plan cards you possess.
  • Your driver’s license or photo identification card.
  • Completed Dentique Dental New Patient Information Form.

Follow-up Visits

Once we have determined a personal treatment plan for you, now will be the time to discuss payment options and any additional questions you might have. We will then schedule you for follow-up visits according to your plan.
